This notice explains the information requested through the quote form on this website and how the Walden Agency uses it to respond to your inquiry.
What you provide
Our quote request form asks for your name, email address, phone number, the type of insurance you’re interested in, and the state where you need coverage. You may also choose to provide a street address, ZIP code, a note, and to opt in to text messages about your request.
How we use it
We use the information you submit to contact you, verify the details needed to prepare a quote, and present available insurance options. Requests are sent to the Walden Agency at jeff@waldenagencyllc.com. We may contact you by phone or email; if you opt in on the form, we may also text you about your quote request.
Please do not submit Social Security numbers, dates of birth, payment information, medical details, or other sensitive information through this form.
